HOSPICE SERVICES
- Hospice Homecare: Hospice care is a specialized approach that focuses on managing the pain and symptoms of chronically, terminally, or seriously ill patients. It also addresses the emotional and spiritual needs of the patient while they remain in their home, a nursing home, or a personal care home.
- Program Services: These services vary by state, counties served, and census goals, and include Hospice Start-up, Setup, and Consulting.
- Hospice Quality Audits/Mock Surveys: Before requesting a formal state survey for permanent licensing and a Medicare Provider Number, we offer the advantage of conducting a thorough audit of the Hospice. Our team will review the Hospice’s policies and procedures, patient utilization, staff charts, quality assurance processes, operations management, and patient care structure.
- Hospice Chart Auditing: When a Hospice’s census increases or staff changes occur, patient documentation may sometimes be compromised. Our team provides percentile chart audits based on patient census to ensure proper Medicare documentation is in place. We also assess whether the correct procedures were followed during Hospice admissions, discharges, revocations, and recertifications.
PERSONAL CARE HOME/ ASSISTIED LIVING
- Housing facilities for people with disabilities, illnesses and who are frail and needy. These facilities provide supervision or assistance with activities of daily living needs, coordination of support services by outside healthcare provider and monitoring of resident activities, health nutrition, medication, and safety needs on 24/hours 7 days a week basis. Our team provides Policy Manual and updates.
PRIVATE HOMECARE PROVIDERS
- Private Homecare Services are purchased by the client directly from the agency or paid on behalf of the client by Community Care Support Program (CCSP) Medicaid Program. Care is generally provided once a day with the aim of keeping frail or disabled people healthy and independent versus full-time help by a live-in nurse. Our team provides Program Manuals, Client & Personnel Files, Individual Care Plans, Accounting Methods, Staff Training and Mock Audits.
COMMUNITY LIVING AGREEMENT (CLA)
CLA's are homecare programs for the developmentally disabled and or the intellectual disabled. The Homes are licensed through DCH and regulated through DBH. Each client has a Now or Comp Medicaid waiver attached to a individual dollar amount that pays for their state services. (CRA) The provider cares for the clients in homes of 4 or less on 24 hour 7 days a week basis, providing support services as outlined in the clients Individual Service Plan created by the provides team. Our agency offers company intent package, business plan, policy & procedures, and talent scouting.
